Since 2017, Lelani Primary and Integrative Health Care and its sister business, High Country House Call, have delivered personalized, high-quality care to the High Country. Lelani focuses on in-office primary care with expertise in integrative health and women’s health, while High Country House Call brings primary care to those unable to leave their homes. Together, they provide compassionate, patient-centered care for the community.
Our Comprehensive Care Services
We offer in-home medical care for established patients, including sick visits, wellness exams, medication management, lab draws, vaccinations, sick care, screenings, and minor procedures. We also collaborate with community services such as CAPs, Services on Aging, Home Health, and Hospice and Palliative Care agencies.
We operate as a comprehensive family practice providing a range of services, including medication management, sick visits and wellness exams, hormone replacement therapy, and contraceptive care. As an established Office-Based Opioid Treatment Center, we support new and established patients during recovery. Our services also include a full-service lab, EKGs, minor surgical procedures, and virtual visits for added convenience. Our providers are certified to perform DOT exams for non-patients, available by appointment.
